Background:The use of oral terbinafine is commonly prescribed for fungal skin infections and for dermatophytosis. The World Health Organization guidelines for the management of fungal skin infections recommend oral terbinafine 1 mg (60 mg daily) for systemic infections and for systemic infections including systemic candidiasis and systemic candidal infections. The aim of this study is to evaluate the safety and efficacy of oral terbinafine versus topical terbinafine for systemic fungal skin infections.
Methods:In this cross-sectional study, data from 1,049 patients with systemic fungal skin infections between 2007 and 2011 was included in this retrospective study. Patients were evaluated for clinical and laboratory findings of skin infections as well as signs of fungal skin infection with the use of oral terbinafine and topical terbinafine. The patients were evaluated for the occurrence of fungal nail infection, dermatophyte fungi, and candidal infections after oral terbinafine therapy. The incidence of candidal infection was compared with those of topical terbinafine. The study was approved by the Ethics Committee of the University of São Paulo (protocol number: 2016/098).
Results:Data from 1,049 patients were included, with an average age of 59 years. The prevalence of fungal nail infection was 22%, dermatophyte fungi were 7.2%, and candidal infections were 3.2%.
Conclusions:Topical terbinafine is generally safe and effective in systemic fungal skin infections. The incidence of dermatophyte fungi was low, but topical terbinafine was more effective in the management of systemic fungal skin infections.
